Even in the physical world, when you see a bad user experience, it's nearly impossible to unsee. My favourite relatively harmless example of this is teabags. I have multiple brands of tea in my home, and not a single one chooses to put the steeping time on the little tag that every single bag is connected to. It is quite literally in my hand as I need that information, yet none of the manufacturers choose to put it on the tag.

The mind boggles.
